Alcohol + Firearms=Death

"KUSA - A 22-year-old woman was shot and killed Tuesday night in what police say was an accidental shooting."

    A 22-year-old woman was shot and killed Tuesday night in what police say was an accidental shooting.

    Notice the use of passive voice in this sentence. Much like "mistakes were made," that sentence near and dear to the hearts of bureaucrats everywhere, it fails to inform us who performed the action.
